What does 1-on-1 Omegle actually mean?
Short answer: a conversation with exactly two people in it, and nobody able to watch. The longer answer is more interesting, because the one-on-one part is the reason anyone still remembers the format.
The term
"1-on-1" describes how many people are in a conversation, not what they are doing in it. One person on each side, no third participant, no audience. When it is used about Omegle specifically, it is a way of saying that the service paired you with a single stranger rather than dropping you into a room full of them.

The shortest spelling, 1v1, comes from gaming — a match between two players. It moved into chat because it says the same thing in three characters.
Why the original worked that way
Omegle launched in 2009 with a very small idea: connect two strangers and get out of the way. There were no profiles to fill in, no friends list and no feed. What you got was a pairing, and when you ended it you got another one.
That constraint is what made it distinctive. Because there was no audience, there was nothing to perform for — and because leaving was free, there was very little cost to being direct. Both of those follow directly from the fact that the room only ever held two people. The service shut down in 2023, but the format did not stop making sense.

This is the distinction the term exists to make. A chat room is open — people come and go, everyone reads everything, and the conversation belongs to whoever talks most. Most people in a busy room never say anything at all.
A one-on-one session removes all of that by removing the audience. There is nobody to impress and nobody to hide behind, which is uncomfortable for about ten seconds and then usually stops being. It also means a bad conversation is genuinely just yours to end, rather than something you have to leave in front of other people.
